ABSTRACT

Objetivo: analisar um instrumento orientador para a Simulação Realística em Saúde utilizado por uma Instituição de Ensino Superior. Método: estudo descritivo, qualitativo com participação de 23 profissionais com Graduação na área da saúde e experiência em simulação realística. A coleta de dados, via questionário on-line, ocorreu no período de março a junho de 2020. Utilizou-se a análise de conteúdo temático-categorial. Resultados: o instrumento contempla as etapas necessárias para o cotidiano do ensino em simulação. A categoria "Itens imprescindíveis para o planejamento da simulação" engloba.: preparo do cenário simulado; atuação dos docentes/facilitadores no briefing e no desenvolvimento do cenário simulado; atuação dos docentes/facilitadores no debriefing; e avaliação da simulação. Conclusão: o desenvolvimento desta pesquisa possibilitou adequação das dimensões do Isimula favorecendo clareza às etapas necessárias à simulação e contribui para apoiar docentes/facilitadores no planejamento, desenvolvimento e avaliação da simulação, em diferentes contextos de atenção à saúde.


Objective: to analyze a guiding instrument for Realistic Simulation in Health used by a Higher Education Institution. Method: this was a descriptive, qualitative study involving 23 professionals with a degree in healthcare and experience in realistic simulation. Data was collected via an online questionnaire between March and June 2020. Thematic-categorical content analysis was used. Results: the instrument covers the necessary steps for day-to-day simulation teaching. The category "Essential items for planning the simulation" includes: preparing the simulated scenario; the role of teachers/facilitators in briefing and developing the simulated scenario; the role of teachers/facilitators in debriefing; and evaluating the simulation. Conclusion: the development of this research enabled the dimensions of Isimula to be adapted, favoring clarity in the stages necessary for simulation and helping to support teachers/facilitators in the planning, development and evaluation of simulation in different healthcare contexts.


Objetivos:analizar un instrumento de orientación para la Simulación Realista en Salud utilizado por una Institución de Enseñanza Superior. Método: estudio descriptivo y cualitativo en el que participaron 23 profesionales con titulación sanitaria y experiencia en simulación realista. Los datos se recogieron a través de un cuestionario online entre marzo y junio de 2020. Se utilizó análisis de contenido temático-categorial. Resultados: el instrumento recoge los pasos necesarios para el día a día de la enseñanza de la simulación. La categoría "Elementos esenciales para la planificación de la simulación" incluye: la preparación del escenario simulado; el papel de los profesores/facilitadores en la información y el desarrollo del escenario simulado; el papel de los profesores/facilitadores en el debriefing; y la evaluación de la simulación. Conclusión: el desarrollo de esta investigación permitió adaptar las dimensiones de Isimula, favoreciendo la claridad en las etapas necesarias para la simulación y ayudando a apoyar a los profesores/facilitadores en la planificación, desarrollo y evaluación de la simulación en diferentes contextos sanitarios.

ABSTRACT

OBJECTIVE To provide reference for improving the rational use of antimicrobial drugs in primary township medical institutions. METHODS Based on the county prescription pre-review center, a team led by anti-infective clinical pharmacists constructed the management mode for the use of antimicrobial drugs in county-level medical communities with clinical pharmacists as the main team by finding out the main problems in the use of antimicrobial drugs in primary township medical institutions, providing feedback on the problems, organizing relevant training for the problems, improving the customization rules of the prescription pre-review software, implementing the automatic interception and pharmacist online prescription review and other measures. Data on the use of antimicrobial drugs were collected and compared in the 15 primary township medical institutions between January-June in 2022 (before the implementation of the mode) and January-June in 2023 (after the implementation of the mode). RESULTS Compared with before the implementation of the mode, the utilization rate of antimicrobial drugs in outpatients of primary township medical institutions decreased from 24.97% before the implementation of the mode to 19.39% after the implementation of the mode; the utilization rate of antimicrobial injection in outpatients decreased from 66.10% to 46.80%; the utilization rate of intravenous drip of antimicrobial drugs in outpatients decreased from 52.33% to 40.35%; the rates of combined use of antimicrobial drugs in outpatients decreased from 12.70% to 8.19%; the reasonable rate of antimicrobial prescribing in outpatients increased from 55.28% to 73.93%. After the implementation of the mode, the proportion of antimicrobial prescriptions for each diagnosis was basically the same as before; the defined daily dose system (DDDs) and proportion of a few antimicrobial drugs changed compared with before according to the anatomical therapeutic chemical classification of drugs, among which DDDs of lincomycin, gentamicin and other drugs declined significantly; DDDs of antimicrobial drugs for each classification was basically the same as before according to AWaRe classification. CONCLUSIONS The management mode of the use of antimicrobial drugs led by anti-infective clinical pharmacists is constructed in the prescription pre-reviewing center of county-level medical communities, which can effectively improve the rational use of antimicrobial drugs in the primary township medical institutions.

ABSTRACT

Este estudo teve como objetivo identificar a variação glicêmica de idosos com diabetes mellitus e COVID-19 em instituições de longa permanência. Foram coletados e analisados prontuários e resultados de exames laboratoriais de 203 idosos residentes em 4 ILPIs no município de Maringá, pertinentes ao período entre 2017 e 2022. Destes, 10 idosos apresentavam diagnóstico de diabetes mellitus, sendo selecionados os 5 que apresentaram diagnóstico de COVID-19 e exames de glicemia detalhada durante o período estudado. Três deles apresentaram aumento na glicemia, enquanto os outros dois praticamente mantiveram os mesmos índices glicêmicos pré e pós-COVID-19. Todos os casos apresentaram manifestações leves da COVID-19 sem necessidade de internação. Os resultados indicam que idosos com índices glicêmicos elevados para a sua respectiva idade podem apresentar índices de elevação da glicêmica quando infectados pelo SARS-CoV-2.


This study aimed to identify the glycemic variation in the elderly with diabetes mellitus and COVID-19 in long stay institutions. Medical records and laboratory test results from 203 elderly residents of 4 LSIE in the municipality of Maringá, pertaining to the period between 2017 and 2022. Of these, 10 were diagnosed with diabetes mellitus, being selected the 5 who were diagnosed with Covid-19 and that had detailed glycemic lab test results from the period in question. Three of them showed glycemic increase, while the other two practically maintained the same glycemic indices before and after Covid-19. All cases had light manifestations of Covid-19, without the need for hospitalization. The results indicate that elderly people with glycemic indices above the normal established for their age may have increased blood glucose when infected with SARS-CoV-2.

Abordou-se a chegada do pensamento lacaniano ao Brasil e a sua divulgação no Congresso Psicanalítico da Banana, marco inaugural no qual se encontra uma expressão nacional da reprodução dos impasses político-institucionais nas escolas lacanianas. Procedeu-se a uma busca no sítio da hemeroteca digital da Fundação Biblioteca Nacional utilizando como descritores o nome do evento e/ou o nome de seus organizadores no período de fevereiro de 1980 a dezembro de 1989. Utilizou-se como referência do trabalho a perspectiva da Escola dos Annales,que considera que a escrita histórica deve se orientar por problemas específicos; a consideração da transferência como marca indelével da pesquisa realizada por psicanalistas pesquisadores; as reflexões de Ricouer sobre as etapas da construção do conhecimento historiográfico. As matérias publicadas apresentam de forma recorrente: as celeumas entre os psicanalistas lacanianos e os filiados à IPA; a busca de reconhecimento dos analistas lacanianos brasileiros pelos franceses; a proposta de discussão da conjuntura nacional por meio da psicanálise; a divergência entre analistas lacanianos brasileiros acerca da tutoria francesa nas instituições surgidas no Brasil. Propôs-se a consideração da dimensão transferencial como possibilidade de construção de uma política emancipatória nas instituições psicanalíticas.


This discussion is about the arrival of Lacanian thinking in Brazil and its dissemination at the Banana Psychoanalytic Congress, an inaugural milestone in which a national expression of the reproduction of political-institutional impasses in Lacanian schools can be found. A search was carried out on the website of the digital newspaper library of the National Library Foundation using as descriptors the name of the event and/or the name of its organizers, from February 1980 to December 1989. What was used as a reference for the work was the perspective of the Annales School which considers that historical writing must be guided by specific problems; the consideration of the transference as an indelible mark of research carried out by researchers of the psychoanalytic field; Ricouer's reflections upon the stages of the construction of the historiographical knowledge. The published articles present recurrently: the controversy between Lacanian psychoanalysts and those affiliated with the IPA; the search for recognition of Brazilian Lacanian analysts by the French; the proposal to discuss the national situation through psychoanalysis; the divergence between Brazilian Lacanian analysts about French tutoring in institutions that emerged in Brazil. It was proposed to consider the transferential dimension as a possibility of building an emancipatory policy in psychoanalytic institutions.


Se ha discutido la llegada del pensamiento lacaniano en Brasil y su difusión en el Congreso Psicoanalítico de Plátano, hito inaugural en el que se encuentra una expresión nacional de los impases político-institucionales en las escuelas lacanianas. Se realizó una búsqueda en la hemeroteca digital de la Fundación Biblioteca Nacional utilizando como descriptores el nombre del evento y/o el nombre de sus organizadores, en el período de febrero de 1980 hasta diciembre de 1989. Se utilizó como referente para el trabajo la perspectiva de la Escuela de los Annales, que considera que la escritura histórica debe guiarse por problemas específicos; la consideración de la transferencia como una marca de la investigación realizada por los psicoanalistas; las reflexiones de Ricouer sobre las etapas de la construcción del conocimiento historiográfico. Los artículos publicados presentan de manera recurrente: la controversia entre los lacanianos y los afiliados a la IPA; la búsqueda del reconocimiento de los lacanianos brasileños por parte de los franceses; la propuesta de discutir la situación nacional a través del psicoanálisis; la divergencia entre lacanianos brasileños sobre la tutoría de francés en instituciones surgidas en Brasil. Se propuso considerar la dimensión transferencial como posibilidad de construcción de una política emancipatoria en estas instituciones.

Resumo O objetivo deste artigo é avaliar as Instituições de Longa Permanência para Idosos (ILPI) brasileiras, segundo o Modelo Teórico Multidimensional Integrado de Qualidade e Atendimento (MIQA), e comparar o desempenho alcançado entre as regiões do país. Estudo ecológico descritivo realizado com dados secundários públicos das ILPI participantes do Censo do Sistema Único da Assistência Social de 2018. Uma Matriz de Avaliação foi construída a partir das variáveis do Censo e do Modelo Teórico MIQA. Parâmetros de qualidade foram empregados para classificar o desempenho das instituições para cada indicador em "incipiente", "em desenvolvimento" ou "desejável. O índice de disparidade foi obtido para cada indicador. Foram analisadas 1.665 instituições. Observaram-se diferenças nos percentuais de ILPI com desempenho "desejável" entre as regiões brasileiras, e a necessidade de aprimoramento na maioria das ILPI em relação à proporção de cuidadores de pessoas idosas, a composição da equipe multiprofissional, a acessibilidade e a oferta de ações de promoção de saúde. Verificou-se a necessidade de apoio governamental para a supressão dos critérios de diferenciações excludentes e para a expansão dos serviços para superar as superlotações.


Abstract This article aims to evaluate the Brazilian Long-Term Institutions for Older People (LTIE), according to the Integrated Multidimensional Theoretical Model of Quality and Service (MIQA), and compare the performance achieved between the regions of the country. Descriptive ecological study carried out with public secondary data from the LTIE participating in the 2018 Census of the Unified Social Assistance System. An Evaluation Matrix was constructed from the Census variables and the MIQA Theoretical Model. Quality parameters were used to classify the institutions' performance for each indicator as "incipient", "developing" or "desirable". The disparity index was obtained for each indicator. 1,665 institutions were analyzed. Differences were observed in the percentages of LTIE with "desirable" performance between Brazilian regions, and the need for improvement in most LTIE in relation to the proportion of caregivers of older people, the composition of the multidisciplinary team, accessibility and supply of health promotion actions. There was a need for government support for the suppression of exclusionary differentiation criteria and for the expansion of services to overcome overcrowding.

ABSTRACT

Objective:To investigate the spiritual health status of nurses in elderly care institutions, analyze the potential categories of spiritual health of nurses in elderly care institutions and explore the influencing factors of different categories.Methods:This study was a cross-sectional survey. From April to July 2022, 352 nurses from 15 elder care institutions in Shandong province were selected to conduct a questionnaire survey on demographic characteristics and spiritual health. Potential profile analysis was used to identify different spiritual health categories, and one-way ANOVA and ordered logistic regression were used to test the differences in population characteristics.Results:There were group heterogeneity in the spiritual health of nurses in elderly care institutions, which were divided into "high level stable type" (34.66%), "medium level type" (44.89%) and "low self transcendence type" (20.45%). Ordered logistic regression showed that age ( OR = 1.054, OR 95% CI: 1.031-1.078, P<0.01), self-assessment health ( OR = 3.034, OR 95% CI: 1.478-6.221, P<0.01), and spiritual learning and training experience ( OR = 2.221, OR 95% CI: 1.370-3.600, P<0.01) were protective factors of spiritual health. Single parent families ( OR = 0.018, OR 95% CI: 0.002-0.197, P<0.01) and working 12 to 16 hours a day ( OR = 0.029, 95% CI: 0.009-0.091, P<0.01), working more than 16 hours a day ( OR = 0.046, 95% CI: 0.018-0.119, P<0.01) were risk factors for spiritual health. Conclusions:There are three potential categories of nurses′ spiritual health in the elderly care institutions, and different categories of nurses have different characteristics. Institutional managers should carry out targeted interventions according to different characteristics to improve their spiritual health.

ABSTRACT

Objective:To explore the mechanism of factors affecting the long-term care needs of disabled elderly in elderly care institutions, and provide basis for taking targeted nursing intervention.Methods:From July to November 2018, convenience sampling method was used to conduct cross-sectional survey of the current situation and influencing factors of long-term care needs of 508 disabled elderly people in 10 elderly care institutions in Baoding City by general information questionnaire, Barthel index, the Long-term Care Needs of the Disabled Elderly in elderly care institutions, the Questionnaire on the Factors Affecting the Long-term Care Needs of the Disabled Elderly in elderly care institutions. SPSS 21.0 was used for correlation analysis and multiple regression analysis, and AMOS 24.0 was used for model fitting, path and mediation effect analysis.Results:The score of long-term care needs of the disabled elderly in elderly care institution was (105.04 ± 21.68) points; the scores of intra-personal, interpersonal, and extra-personal factors were (16.93 ± 5.20) points, (10.51 ± 3.39) points, (19.80 ± 6.24) points, respectively. Intra-individual factors, interpersonal factors, extra-individual factors and care needs were all positively correlated ( r values were 0.222-0.645, P<0.05). Intra-individual, inter-personal and extra-individual factors could directly affect the long-term care needs of the disabled elderly ( β=0.455, 0.527, 0.403); intra-individual and inter-personal factors had independent mediating effects and chain mediating effects between extra-individual factors and the long-term care needs of the disabled elderly, and the effects were significant, with effect value of 0.078, 0.095 and 0.023, accounting for 15.15%, 18.45% and 4.47% of the total effect, respectively. Conclusions:Each influencing factor has a strong ability to predict and explain the long-term care needs of the disabled elderly, and nursing staff could meet the care needs of the disabled elderly according to different influencing factors.

Under the national policy background of traditional Chinese medicine(TCM)registration review and approval reform, TCM preparations in medical institutions(hereinafter referred to as medical institution preparations) have human use experience, conform to the characteristics of TCM clinical practice, and have advantages in the research and development of innovative Chinese medicines. Therefore, the research and development mode based on clinical experience prescription-medical institution preparations-innovative Chinese medicines has attracted widespread attention from the industry. However, in the process of development and use of medical institution preparations, there are generally clinical problems that restrict their transformation into innovative Chinese medicines, resulting in a relatively weak collection basis of human use experience and insufficient clinical evidence for supporting the research and development of new TCM medicines. In this paper, on the basis of sorting out the supporting regulations and relevant technical requirements of human use experience, and analyzing the clinical problems restricting the transformation of medical institution preparations, it provides suggestions for medical institutions to carry out high-quality research on human use experience of preparations in the process of TCM clinical practice from the aspects of continuously exploring clinical value, improving the construction of information system, focusing on the TCM clinical practice and giving full play to the advantages of discipline cooperation. By realizing the whole life cycle management of medical institution preparations based on three-combination evaluation evidence system, we can promote the transformation of medical institution preparations into innovative Chinese medicines.

A evasão universitária é um fenômeno complexo, que se caracteriza pela decisão ativa do abandonar o curso. Este estudo teve como objetivo revisar a literatura científica (método PRISMA) acerca da evasão universitária dos últimos dez anos (2010 e 2019) no Portal dos Periódicos da Coordenação de Aperfeiçoamento de Pessoal de Nível Superior. Dos 18 artigos analisados, as maiores frequências foram de estudos transversais, publicados entre 2016 e 2019, em periódicos das áreas de Psicologia e da Educação. Maiores frequências de investigações com discentes nos cursos da área "Negócios, administração e direito" e de instituições públicas. Os construtos associados à evasão foram vivências acadêmicas, adaptabilidade de carreira, satisfação com a instituição e autoeficácia na formação superior. Questionários sem estudos psicométricos anteriores foram os instrumentos mais utilizados para mensurar motivos para evasão. Outras discussões e recomendações são apresentadas no decorrer do artigo


University dropout is a complex phenomenon, characterized by the student's active decision to drop out of the course. This study aimed to review the scientific literature (PRISMA method) about university dropouts in the last ten years (2010 and 2019) in the Journal Portal of the Coordination for the Impro-vement of Higher Education Personnel. Of the 18 articles analyzed, the highest frequencies were cross-sectional studies, published between 2016 and 2019, in journals in the fields of Psychology and Education. Higher frequency of investigations with students in courses in the area "Business, administration and law" and in public institutions. The constructs associated with dropout were academic experiences, career adaptability, satisfaction with the institution and self-efficacy in higher education. Questionnaires without previous psychometric studies were the instruments most used to measure reasons for dropout. Other discussions and recommendations are presented throughout the article


La deserción universitaria es un fenómeno complejo, caracterizado por la decisión activa del alumno de abandonar la asignatura. Este estudio tuvo como objetivo revisar la literatura científica (método PRISMA) sobre la deserción universitaria en los últimos diez años (2010 y 2019) en el Portal de Revistas de la Coordinación para la Perfeccionamiento del Personal de Educación Superior. De los 18 artículos analizados, las frecuencias más altas fueron estudios transversales, publicados entre 2016 y 2019, en revistas del ámbito de la Psicología y la Educación. Mayor frecuencia de investigaciones con estudiantes en cursos del área "Negocios, administración y derecho" y en instituciones públicas. Los constructos asociados a la deserción fueron experiencias académicas, adaptabilidad a la carrera, satisfacción con la institución y autoeficacia en la educación superior. Los cuestionarios sin estudios psicométricos previos fueron los instrumentos más utilizados para medir los motivos de abandono. Otras discusiones y recomendaciones se presentan a lo largo del artículo

ABSTRACT

Resumo Este estudo analisa a dissolução da cooperação institucional, com duração de aproximadamente uma década, entre o Ministério da Saúde e um conjunto de instituições acadêmicas para efetivação do Programa Nacional de Melhoria do Acesso e da Qualidade da Atenção Básica (PMAQ-AB) no âmbito do Sistema Único de Saúde (SUS). Esta pesquisa exploratória e qualitativa, consiste em um estudo de caso que utiliza pressupostos do Neoinstitucionalismo Histórico para orientar a análise. Os dados foram coletados por meio de pesquisa documental, bibliográfica e entrevistas semiestruturadas com atores-chave. A partir da análise de conteúdo, foram evidenciadas três categorias analíticas: o fim do PMAQ-AB - processo histórico, contexto político-institucional e atores; o fim da cooperação institucional - processo de interação governo-academia; e percepções dos atores sobre o encerramento da cooperação institucional. O fim da cooperação institucional se deu em um contexto de mudança de governo na esfera federal, dentro de uma nova ordem político-institucional do SUS, e influenciado por fatores históricos e pelas práticas dos gestores públicos na condução dos processos políticos. um intercâmbio interinstitucional governo-academia com vistas ao fortalecimento da gestão da saúde e apoio ao desenvolvimento institucional do SUS, considerando as adversidades impostas ao campo da saúde e da educação pública no Brasil atualmente.


Abstract This study analyzes the dissolution of the institutional cooperation, lasting approximately a decade, between the Ministry of Health and a set of academic institutions for the implementation of the National Program for Improving Access and Quality of Primary Care (PMAQ-AB) in the scope of the Brazilian Nacional Health System (SUS). This exploratory, qualitative research consists of a case study that uses assumptions of Historical Neoinstitutionalism to guide the analysis. Data were collected by documentary and bibliographic research and semi-structured interviews with key actors. From the content analysis, three analytical categories were evidenced: end of the PMAQ-AB - historical process, political-institutional context and actors; end of the institutional cooperation - government-academy interaction process; and actors' perceptions about the end of institutional cooperation. The end of institutional cooperation happened in a context of change of government at the federal level, within a new political-institutional order of SUS and was influenced by historical factors and the practices of public managers and in the conduct of political processes. An inter-institutional government-academy exchange with a view to strengthening health management and supporting the institutional development of the SUS is a future expectation, considering the adversities imposed on the field of health and public education in Brazil today.

ABSTRACT

ABSTRACT Objectives: to analyze undergraduate nursing course coordinators' perception about nursing training in palliative care. Methods: a descriptive study, with a qualitative approach and thematic content analysis, carried out with coordinators of nursing courses in Higher Education Institutions in Rio Grande do Norte. Results: three thematic categories emerged: Nursing training in palliative care; Potentialities for teaching palliative care; and Challenges of teaching in palliative care. The coordinators described as potentialities: transversality, theoretical and practical approach, optional subject, university extensions, interdisciplinarity and transdisciplinary approach, and as challenges: biomedical model in health education and insufficient professor training. Final Considerations: palliative care teaching in the researched institutions in the nursing education process is approached in an incipient and fragmented way, and almost always without having a specific curricular component on the subject, being present as one of its contents.


RESUMEN Objetivos: analizar la percepción de los coordinadores de cursos de graduación en enfermería sobre la formación de enfermería en cuidados paliativos. Métodos: estudio descriptivo, con abordaje cualitativo y análisis de contenido temático, realizado con coordinadores de cursos de enfermería en Instituciones de Educación Superior de Rio Grande do Norte. Resultados: surgieron tres categorías temáticas: Formación de enfermeros en cuidados paliativos; Potencialidades para la enseñanza de cuidados paliativos; y Desafíos de la enseñanza en cuidados paliativos. Los coordinadores describieron como potencialidades: transversalidad, enfoque teórico y práctico, materia optativa, extensiones universitarias, interdisciplinariedad y abordaje transdisciplinar, y como desafíos: modelo biomédico en educación para la salud e insuficiente formación docente. Consideraciones Finales: la enseñanza de los cuidados paliativos en las instituciones investigadas en el proceso de formación de enfermería es abordada de forma incipiente y fragmentada, y casi siempre sin tener un componente curricular específico sobre el tema, estando presente como uno de sus contenidos.


RESUMO Objetivos: analisar a percepção dos coordenadores de curso de graduação em enfermagem sobre a formação do enfermeiro em cuidados paliativos. Métodos: estudo descritivo, de abordagem qualitativa e análise de conteúdo temática, realizado com coordenadores dos cursos de enfermagem em Instituições de Ensino Superior do Rio Grande do Norte. Resultados: emergiram três categorias temáticas: Formação do enfermeiro em cuidados paliativos; Potencialidades para o ensino dos cuidados paliativos; e Desafios do ensino em cuidados paliativos. Os coordenadores descreveram como potencialidades: transversalidade, abordagem teórica e prática, disciplina optativa, extensões universitárias, interdisciplinaridade e abordagem transdisciplinar, e como desafios: modelo biomédico na formação em saúde e formação docente insuficiente. Considerações Finais: o ensino dos cuidados paliativos nas instituições pesquisadas no processo de formação do enfermeiro é abordado de forma incipiente e fragmentado, e quase sempre sem ter um componente curricular específico sobre o tema, estando presente como um de seus conteúdos.

O objetivo do estudo foi investigar o impacto das variáveis habilidades sociais, resolução de problemas sociais, automonitoria, autoeficácia e coping na adaptação acadêmica em estudantes de instituições de ensino superior públicas e privadas. Participaram 637 estudantes de ambos os sexos, sendo 36,5% (115) homens de instituições públicas e 22,3% (72) de instituições privadas, com idade variando entre 18 e 38 anos (M=24,7; DP=6,3), de diferentes graduações. Foram utilizados o Inventário de Resolução de Problemas Sociais, o Inventário de Habilidades Sociais, a Escala de Automonitoria, a Escala de Autoeficácia Acadêmica de Estudantes do Ensino Superior, o Inventário de Estratégias de Coping e o Questionário de Vivências Acadêmicas-reduzido. A autoeficácia na gestão acadêmica (40,9%) e a autoafirmação na expressão de afeto positivo (13,7%) apresentaram maior impacto para os estudantes de instituições públicas e privadas, podendo contribuir com possíveis intervenções no processo de adaptação ao ensino superior. Pesquisas prospectivas podem investigar questões relacionadas a dados sociodemográficos.(AU)


The aim of the study was to investigate the impact of the variables Social Skills, Resolution of Social Problems, Self-monitoring, Self-efficacy and Coping on Academic Adaptation in students from public and private higher education institutions. 637 students of both sexes participated, being 36.5% (115) men from public institutions and 22.3% (72) from private institutions, aged between 18 to 38 years (M = 24.7; SD = 6.3), of different grades. The Social Problem Solving Inventory, the Social Skills Inventory, the Self-Monitring Scale, the Higher Education Students' Academic Self-Efficacy Scale, the Coping Strategies Inventory and the Academic Experiences-Reduced Questionnaire were used. Self-efficacy in Academic Management (40.9%) and Self-affirmation in the Expression of Positive Affection (13.7%) had a greater impact on students from public and private institutions, which may contribute to possible interventions in the process of adapting to Higher Education. Prospective research can investigate issues related to sociodemographic data.(AU)


El objetivo del estudio fue investigar el impacto de las variables Habilidades sociales, Resolución de problemas sociales, Autocontrol, Autoeficacia y Afrontamiento en la adaptación académica en estudiantes de instituciones de educación superior públicas y privadas. Participaron 637 estudiantes de ambos sexos, siendo 36,5% (115) hombres de instituciones públicas y 22,3% (72) de instituciones privadas, con edades entre 18 y 38 años (M = 24,7; SD = 6,3), de diferentes grados. ABSTRACT

Abstract What are the effects of extractive informal institutions on the horizontal accountability process in a developing democracy? This paper presents evidence about the harmful effects of extractive informal institutions on horizontal accountability within subnational governments in Brazil. After three decades of free and competitive elections, the institutional design of oversight institutions for state governments has hardly changed. We explore the hypothesis that more extractive informal institutions, measured by the level of income inequality, is associated with decreasing transparency levels. Given its extensive social, political, and economic diversity embedded in an overall centralized formal institutional framework, Brazil provides an appropriate setting to test the hypothesis that extractive informal institutions responsible for increased income inequality can undermine horizontal accountability in new democracies.


Resumen ¿Cuáles son los efectos de las instituciones informales extractivas sobre el proceso de accountability horizontal en una democracia en desarrollo? Este artículo presenta evidencia sobre los efectos dañinos de las instituciones informales extractivas sobre la accountability horizontal dentro de los gobiernos subnacionales en Brasil. Después de tres décadas de elecciones libres y competitivas, el diseño institucional de las instituciones de supervisión de los gobiernos estatales apenas ha cambiado. Exploramos la hipótesis de que las instituciones informales más extractivas, medidas por el nivel de desigualdad de ingresos, están asociadas con niveles decrecientes de transparencia. Dada su amplia diversidad social, política y económica integrada en un marco institucional formal, general y centralizado, Brasil proporciona un entorno apropiado para probar la hipótesis de que las instituciones informales extractivas, responsables de una mayor desigualdad de ingresos, pueden socavar la accountability horizontal en las nuevas democracias.


Resumo Quais são os efeitos das instituições informais extrativas no processo de accountability horizontal numa democracia em desenvolvimento? Este artigo apresenta evidências sobre os efeitos nocivos das instituições informais extrativistas na accountability horizontal dos governos subnacionais no Brasil. Após três décadas de eleições livres e competitivas, o desenho institucional das instituições de fiscalização dos governos estaduais praticamente não mudou. Exploramos a hipótese de que instituições informais mais extrativas, medidas pelo nível de desigualdade de renda, estão associadas a níveis decrescentes de transparência. Dada a sua extensa diversidade social, política e econômica, inserida num quadro institucional formal centralizado, o Brasil oferece um cenário apropriado para testar a hipótese de que as instituições informais extrativas responsáveis pelo aumento da desigualdade de rendimentos podem minar a accountability horizontal nas novas democracias.

ABSTRACT

O objetivo do presente estudo foi analisar as condições estruturais disponibilizadas para o ensino das disciplinas de Avaliação Psicológica nos cursos de Psicologia das Instituições de Ensino Superior (IES) do Ceará. Obteve-se autorização de oito instituições para realizar a pesquisa e, tendo como base um roteiro de observação, foram analisados aspectos de material e equipamento, infraestrutura e Serviço de Psicologia. Os principais resultados indicaram que as instituições possuíam uma variedade mínima de 20 testes psicológicos, os alunos assinavam um termo para alugar os testes e as IES desenvolviam atividades para a comunidade. Entretanto, também foi observado a inexistência da testagem informatizada, poucas instituições contavam com testotecas e os funcionários responsáveis pelo serviço e material da avaliação psicológica, em sua maioria, não eram formados em Psicologia. Pode-se concluir que o panorama das instituições cearenses observadas é promissor, mas deve-se continuar em busca de melhorias


The aim of the present study was to analyze the structural conditions made available for the teaching of Psychological Assessment disciplines in the Psychology courses of Higher Education Institutions in Ceará. Authorization was obtained from eight institutions to carry out the research and, based on an observation script, were analyzed aspects of material and equipment, infrastructure and Psychology Service. The main results indicated that the institutions had a minimum range of 20 psychological tests, students signed a term to rent the tests and the institutions developed activities for the community. However, it was also observed that there was no computerized testing, few institutions had "testotecas" and the employees responsible for the service and psychological assessment material were mostly not graduate in Psychology. It can be concluded that the scenario of the institutions in Ceará observed is promising, but one must continue to seek improvements


El objetivo del presente estudio fue analizar las condiciones estructurales disponibles para la enseñanza de asignaturas de Evaluación Psicológica en los cursos de Psicología de las Instituciones de Educación Superior en Ceará. Se obtuvo autorización de ocho instituciones para llevar a cabo la investigación y, en base a un guion de observación, se analizaron aspectos de material y equipo, infraestructura y Servicio de Psicologia. Los principales resultados indicaron que las instituciones tenían un rango mínimo de 20 tests psicológicos, los estudiantes firmaron un plazo para alquilar el tests y las IES desarrollaron actividades para la comunidad. Sin embargo, también se observó que no había tests computarizados, pocas instituciones tenían "testotecas" y los empleados responsables del servicio y el material de evaluación psicológica en su mayoría no estaban graduado en psicología. Se puede concluir que el escenario de las instituciones observadas en Ceará es prometedor, pero hay que seguir buscando mejoras

ABSTRACT

Objetivo: conhecer os aspectos implicados na confortabilidade das mulheres hospitalizadas na unidade materno-infantil, na perspectiva dos profissionais de enfermagem. Método: pesquisa qualitativa de caráter exploratório e descritivo realizada em uma unidade materno-infantil, com 21 profissionais de enfermagem. A coleta de dados ocorreu por entrevista semiestruturada e, posteriormente, os dados foram submetidos a análise temática. Resultados: dentre os aspectos que contribuem para a confortabilidade destacam-se: mobiliário, serviço de hotelaria, métodos não farmacológicos para alívio da dor, respeito às escolhas da parturiente, direito ao acompanhante e disponibilidade da equipe. Como aspectos que dificultam a confortabilidade evidenciou-se a necessidade de adequação da estrutura física com a adoção de quartos pré-parto, parto e pós-parto, e espaços que permitam a vivência do luto em situações de óbito fetal. Considerações finais: a confortabilidade não está relacionada somente aos aspectos estruturais, materiais ou arquitetônicos, mas envolve as relações e interações estabelecidas na unidade materno-infantil.


Objetivo: conocer los aspectos involucrados en el confort de la mujer hospitalizada en la unidad materno-infantil, desde la perspectiva de los profesionales de enfermería. Método: investigación cualitativa exploratoria y descriptiva; realizada en una unidad materno-infantil, con 21 profesionales de enfermería. La recolección de datos ocurrió a través de entrevistas semiestructuradas, y posteriormente los datos fueron sometidos al análisis temático. Resultados: entre los aspectos que contribuyen al confort, se destacan: mobiliario, servicio hotelero, métodos no farmacológicos para el alivio del dolor, respeto a las elecciones de la parturienta, derecho a acompañante y disponibilidad del equipo. Como aspectos que dificultan el confort, se evidenció la necesidad de adecuar la estructura física con la adopción de salas de preparto, parto y posparto, y espacios que permitan la vivencia del duelo en situaciones de muerte fetal. Consideraciones finales: el confort no sólo está relacionado con aspectos estructurales, materiales o arquitectónicos, sino que involucra las relaciones e interacciones que se establecen en la unidad materno-infantil.


Objective: to know the aspects involved in the comfortability of women hospitalized in the maternal and child unit, from the perspective of nursing professionals. Method: qualitative exploratory and descriptive research conducted in a maternal-child unit with 21 nursing professionals. Data collection occurred by semi-structured interview, and subsequently the data were submitted to thematic analysis. Results: among the aspects that contribute to comfortability stand out: furniture, hospitality service, non-pharmacological methods for pain relief, respect for the choices of the parturient, right to the companion and availability of the team. As aspects that hinder comfortability, it was evidenced the need to adapt the physical structure with the adoption of pre-delivery, delivery and post-delivery rooms, and spaces that allow the experience of grief in situations of fetal death. Final considerations: comfortability is not only related to structural, material or architectural aspects, but involves the relationships and interactions established in the maternal-child unit.

Resumo Objetivo O presente estudo teve como objetivo identificar as medidas sanitárias adotadas pelas universidades públicas federais no contexto da pandemia da COVID-19. Métodos Trata-se de uma pesquisa quali-quantitativa, exploratória e descritiva feita por meio da análise documental dos protocolos, diretrizes, portarias e cartilhas confeccionadas no âmbito universitário federal do Brasil. Quanto aos critérios de inclusão foram o documento estar disponível no site de cada universidade federal no período da coleta e a livre consulta do acervo pela internet. O período de coleta foi de março de 2020 a novembro de 2021. Resultados Foram encontrados 51 documentos. As universidades do Nordeste e Sudeste foram responsáveis por 46,4% das publicações totais das universidades federais do Brasil. Conclusão Diante das medidas adotadas pelas Instituições Federais de Ensino Superior, foi perceptível a congruência em tentar estabelecer rotinas e procedimentos padronizados que fossem capazes de controlar e minimizar a disseminação da COVID-19 dentro da universidade.

Los criterios de inclusión fueron la disponibilidad del documento en el sitio web de cada universidad nacional en el período de la recopilación y la libre consulta del acervo por internet. El período de recopilación fue de marzo de 2020 a noviembre de 2021. Resultados Se encontraron 51 documentos. Las universidades del nordeste y del sudeste fueron responsables por el 46,4 % de las publicaciones totales de las universidades nacionales de Brasil. Conclusión Ante las medidas adoptadas por las instituciones nacionales de educación superior, se percibió congruencia para intentar establecer rutinas y procedimientos estandarizados que fueran capaces de controlar y minimizar la diseminación de COVID-19 dentro de la universidad.


Abstract Objective The present study aimed to identify the health measures adopted by federal public universities in the context of the COVID-19 pandemic. Methods This is a qualitative-quantitative, exploratory and descriptive research carried out through documental analysis of protocols, guidelines, ordinances and booklets made in the federal university scope of Brazil. As for the inclusion criteria, documents must be available on the website of each federal university during the collection period and free consultation of the collection on the internet. The collection period was from March 2020 to November 2021. Results We found 51 documents. Northeastern and southeastern universities were responsible for 46.4% of the total publications of federal universities in Brazil. Conclusion Considering the measures adopted by Higher Education Federal Institutions, the congruence in trying to establish standardized routines and procedures that were able to manage and minimize the spread of COVID-19 within the university was noticeable.

ABSTRACT

Objective To investigate the prognosis of middle-aged and elderly patients with aspiration pneumonia and related influencing factors in integrated medical and elderly care institutions. Methods A total of 604 elderly patients with aspiration pneumonia were selected from integrated medical and elderly care institutions from January 2016 to December 2020. The prognosis of the patients was counted, and the prognostic factors were analyzed by univariate and multivariate logistic regression equations. The receiver operating characteristic (ROC) curve and area under the curve (AUC) were used to analyze the predictive value of logistic regression model for the prognosis of elderly patients with aspiration pneumonia. Results Univariate analysis showed that Alzheimer's disease, severe pneumonia, ability of daily living (ADL), and the number of rescues were related to the prognosis of elderly patients with aspiration pneumonia (P12.97), the predictive value was the best, with the predictive sensitivity being 83.87%, and the specificity being 84.15%. Conclusion Alzheimer's disease, severe pneumonia, moderate and severe dependence in the ability of daily living and rescue times ≥ 2 are related to the prognosis of middle-aged and elderly patients with aspiration pneumonia in the integrated medical and elderly care institutions, which can be used as a prognostic plan for clinical treatment and nursing management. At the same time strengthening the intervention of patients with the above characteristics will help to promote the improvement of prognosis.

ABSTRACT

Objective To comprehensively analyze and understand the status of medical resources of radiological diagnosis and treatment in Suzhou, China, and to provide a basis for the health administration departments to reasonably plan and allocate medical radiation resources. Methods The radiological diagnosis and treatment institutions were registered on the radiation health information platform of Jiangsu province, and information was entered as required. Results There were totally 793 radiological diagnosis and treatment institutions at all levels in Suzhou, including 22 (6.04%) tertiary institutions. There were 2208 radiological diagnosis and treatment equipment. The number of X-ray diagnosis and treatment equipment per million people in Suzhou was 205.40. However, there was no class A large-scale medical equipment. Conclusion Compared with 2005, the numbers of radiological diagnosis and treatment institutions and equipment in Suzhou increased significantly. However, government departments at all levels should strengthen overall regulation and control to improve the rational allocation of high-tech medical resources.

OBJECTIVE To investigate the present equipment and management situation of narcotic drugs in primary healthcare institutions from Qiandongnan prefecture of Guizhou province. METHODS The questionnaire survey was conducted among pharmacy department heads and medical staff from primary healthcare institutions in Qiandongnan prefecture of Guizhou province. Descriptive statistical analysis was conducted on the survey results. RESULTS Of 251 healthcare institutions in this survey, 29 healthcare institutions were equipped with narcotic drugs, accounting for 11.55%. The reasons for the narcotic drugs unequipped were mainly as follows: insufficient attention, no storage conditions for narcotic drugs, complex program of narcotic drug management, small amount usage and so on. Among the 29 primary healthcare institutions equipped with narcotic drugs, all of them did not monitor patient usage, accounting for 100%; 29 healthcare institutions did not implement a return visit or follow-up every 3 months, accounting for 100%. CONCLUSIONS The health administration departments should strengthen the administration of narcotic drugs in primary healthcare institutions. At the same time, training on standardized management and clinical rational application of narcotic drugs for medical staff in primary healthcare institutions should be enhanced by the health administrative department.

ABSTRACT

To provide prevention and control strategies of occupational exposure risks to cytotoxic drug in medical institutions, improve the awareness of protection among medical staff,and reduce potential occupational exposure risks,the Guidelines for the Prevention and Control of Occupational Exposure Risk to Cytotoxic Drugs in Medical Institutions is developed. Based on the World Health Organization Guidelines Development Manual,the exposure risk issues of the cytotoxic drug collected from the time that it entered the hospital to the several stages after delivery to hospital,such as transportation,receipt,storage,unpacking,dispensing,use of finished products,and waste disposal. Delphi method is used to construct identification of clinical issues,and evidence-based research method is used to develop relevant evidence. Quality evaluation is conducted by using the recommended GRADE method. The consensus is reached on the recommendation opinions and evidence levels through expert consensus method. By combining engineering controls,administrative controls and personal protective equipment at different levels,a graded control approach is established. A total of 37 clinical issues are identified,resulting in 36 recommendations. This guideline provides reference and supplementation for the formulation of cytotoxic drug prevention and control measures in medical institutions.
